Patient Rights & Responsibilities


As our patient, you have the right to:

  • Be treated in a caring and polite way without discrimination as to age, race, color, religion, sex, national origin, disability, sexual orientation or ability to pay.

  • Know all the facts we have about your illness or injury. Your referring physician will give these facts to you after receiving the report from your exam.

  • Know the names of the people taking care of you, as well as how they will help you or provide care for you.

  • Say yes to examinations. You also have the right to say no to or refuse examinations.

  • Be examined in private by your doctor or other health care giver, and you have the right to talk to your doctor in private.

  • Receive a complimentary CD or films of your images for your next appointment with your referring physician. You may request additional copies for a reasonable fee.

  • Expect that we will do our best to get back to you as quickly as possible when you ask us to do something.

  • Tell us when something is seriously wrong. If you present a complaint, your care will not be adversely affected. Also, if you have a problem that you cannot solve with your doctor, nurse or other caregiver, please contact Melanie Myers, center manager, at MMyers2@AllianceImaging.com or (219) 769-5000, ext. 26.


To ensure the best possible care, you have the responsibility to:

  • Be honest with us and tell us all you know about your present condition, including other studies you've had and anything else about your health that would help us.

  • Tell us if you don't understand what our staff is saying to you or if you don't understand what they're asking you to do. Also, tell us if you think you won't be able to do what is asked of you during your exam.

  • Respect the right of other patients to receive medical care without being disturbed.

  • Observe the "no smoking" rules of this practice.

  • Give our staff all the information they need regarding payment for your medical care.
